Activating Patients : Turning Challenges into Opportunities to Improve Patient Outcomes
An informed and activated patient understands the disease process, and realizes his/her role as the daily self manager in their health care. The provider is seen as a guide on the side, not the sage on the stage. This session will focus on the challenges health care providers face in activating patients in their health care sharing specific areas of challenge and how to turn those challenges into opportunities to improve patient outcomes.

PBRN: Where Practice and Research Meet
PBRNs (Practice-Based Research Networks) are groups of primary care clinicians and practices working together to answer community-based health care questions and translate research findings into practice. This session will focus on the work of MaRC (Maryland Research Collaborative) whose mission is to improve quality of care and patient outcomes as well as address health disparities by promoting primary care research in Community Health Center practice settings, in partnership with University of Maryland School of Medicine academic researchers.

Mobilizing Community Support to Assist CHC’s in Patient Care
DHOW (Maryland Department of Health) and DHSS (Delaware Health and Social Services) have proven to maintain long-standing partners in the community and Community Health Centers. This session will focus on some of the partnering opportunities for health centers using the services of their local department of health.

FQHC’s and Using a Contract Pharmacy
There is a national shortage of Pharmacists, and competing with the big chains to get them is a challenge for in-house pharmacies. The alternative of using a contract pharmacy to deliver 340B drugs is also a challenge, but with the right systems and tools you can overcome them and succeed in the contract pharmacy model. This session will focus on case studies of successes and failures in the contract pharmacy model. It will review 340B regulations, proposed new legislation and the impact of the pharmacy system on patient safety.

Immunization Updates / Conversations Maps in Treating DM Patients
This is a dual-clinical update session. The first part of this session will focus on the June 30, 2005 the CDCs Advisory Committee on Immunization Practices (ACIP) updated recommendations for the routine use of Tdap vaccines in adolescents aged 11–18 years. A portion of this session will focus on strategies to support proper patient immunization.
The second half of this session will introduce the use of Conversation Maps, a diabetes education program. This program is designed to enlighten, inspire and empower patients by helping them make the necessary lifestyle modifications leading to improve self management care for their diabetes.

UDS/HDC: Making the Connection Between What is Required and Improving Patient Outcomes
Many changes have occurred regarding requirements of CHC’s in their clinical quality programs. This session will focus on the work of the health disparities collaborative and the integration of that work into the work of CQI focusing on the new UDS clinical measures. Updates to UDS changes will be reviewed as well as strategies to improve on the measures collected.
